  1. Regularly travels to various program sites and centers to complete comprehensive assessments according to program guidelines, and formulates diagnostic impression, by conducting client and/or family interviews, reviewing substance abuse and mental health treatment history and conferring with staff and referral sources, and so forth.  Determines client’s appropriateness for treatment, level of care required, strengths and problems, potential dual diagnosis, and requirements for other ancillary services.  May obtain information by telephone to conduct preliminary admission screening.


  1. Completes admission forms, and prepares client chart by accurately recording pertinent information.  Obtains necessary consent signatures from client or guardian.  Makes copies of forms and distributes to business office, medical department, and client chart as appropriate.


  1. Prepares initial client treatment plan on a timely basis, in accordance with assessment and evaluation, which could include assigning client to appropriate group and counselor; and identifying need for referral resources or services such as parenting classes, job search support, education, and medical care.


  1. Reports admission activities and progress to supervisor as required.  Maintains records and charts in accordance with organizational, regulatory, accreditation, and contractual standards.


  1. Serves as professional resource, and provides clinical supervision to other Counselors as needed.  Audits client records prepared by others to ensure compliance with required documentation and service delivery.  Provides assessments of special needs clients, and advice and guidance necessary to resolve complex case problems.  Demonstrates and/or instructs on appropriate procedures, modalities and techniques.


  1. As requested and appropriate, develops and presents introductory and continuing education programs on subjects related to assigned specialty for other Counselors or Technicians.  Trains and oversees assigned Interns.


  1. Explains treatment plan and objectives, orients client to treatment program and facilities, and explains policies and rules.  Provides client with relevant information such as meeting schedules and locations.


  1. Develops and maintains regular contact (usually by telephone) with family members and/or significant others; as well as variety of outside agencies such as social services, corrections, protective services, probation or parole; in order to facilitate assessment process, and complete necessary treatment follow-up and case management.


  1. Performs initial case management in order to facilitate program admission and retention; which includes encouraging and motivating clients’ participation and commitment to treatment, directing clients to additional services as needed, following up with various agencies and insurance carriers to resolve admission problems, following up on appointments missed by clients, providing crisis intervention and so forth.


  1. Performs duties of Counselor such as providing group and individual counseling, conducting education session, and completing related treatment charting and documentation, as necessary in response to workload or staffing shortages.


  1. Serves on variety of department or site committees necessary to plan and evaluate treatment programs and client services.  Contributes to continuous improvement of treatment protocols, procedures and results.


  1. Maintains and enhances knowledge and expertise through appropriate educational and organizational activities.


  1. Valid local driver’s license, insurance and good driving record required.
